We are looking for individuls with experience in working with High School Students with disabilities in planning out their future. The project works with individuals who are currently involved in the transition process from high school to whatever they choose for themselves next. The project uses person centered planning techniques to provide the individual and the family with realistic goals to help make the process an easy transition.
